A yellow blip means that the sub won't reach the destination. It will probably either be targeted by a pirate/mutual annihilation will occur (these are the most common reasons). There are several other ways you can get a yellow blip, such as the sub being in a martyr blast radius, or a sub running out of drillers as it is getting shot by a sentry, etc.

And the follow up is...

A red blip means you'll reach the destination, but lose. (Given current known conditions.)
